This tool gives a PDF the navigation it should have shipped with: a bookmark tree in the reader sidebar, a printed contents page at the front, or both.
The hard part is that a PDF has no idea what a heading is. It holds marks at coordinates on a page, and the fact that one line is a chapter title and the next is a paragraph exists only in the eye of whoever looks at it. So the headings are found by measurement: the body text size is worked out across the whole document, and any line noticeably larger and short enough to be a title becomes a candidate.
That is a good guess and not a fact, which is why the list appears on the page for you to correct. Untick what is not a heading, fix a title that was split in two, set the levels so sections sit under chapters, add a line the document set in ordinary type. Then it gets written, and what gets written is what you approved.
Bookmarks change nothing about the pages themselves, so they are safe on any document. A printed contents page is more involved: it adds pages at the front, which moves everything after it, and the entries are adjusted so their links still land in the right place. Both facts are stated here rather than discovered afterwards.
